National Provider Identifier (NPI)

Notice:

All High Fidelity Wraparound (HFWA) providers, both individuals and agencies, are required to obtain a National Provider Identifier (NPI) before applying to join the HFWA network.  If you already have a NPI number, do not apply for another one.

Go to the National Provider Plan & Enumeration System (NPPES) to apply.

Type 1:

Individual providers and agency/group members will obtain an individual NPI.

Type 2:

Agencies/groups will also obtain an organization NPI.

What you need to know when applying for an NPI

Prepare with the information that will be needed:

Information required for individual providers

  • Provider Name
  • SSN (or TIN)
  • Provider Date of Birth
  • Country of Birth
  • State of Birth
  • Provider Gender
  • Mailing Address
  • Practice Location Address and Phone Number
  • Taxonomy (Provider Type) – 171M00000X  Care Mgr Care Coordinator
  • State License Information (as applicable)
  • Contact Person Name
  • Contact Person Phone Number and E-mail

Information required for organizations

  • Organization Name
  • Employer Identification Number (EIN)
  • Name of Authorized Official for the Organization
  • Phone Number of Authorized Official for the Organization
  • Organization Mailing Address
  • Practice Location Address and Phone Number
  • Taxonomy (Provider Type) – 171M00000X  Care Mgr Care Coordinator
  • Contact Person Name
  • Contact Person Phone Number and E-mail

Common mistakes that will hold up your application

  • Make sure the name you use on the application is the same as the one on your Social Security Card.
  • When they ask about an address – if at all possible choose “accept standardized.”
  • You will use the same address with NPPES, WY Medicaid and Magellan.
