Type 2:
Agencies/groups will also obtain an organization NPI.

Common mistakes that will hold up your application
- Make sure the name you use on the application is the same as the one on your Social Security Card.
- When they ask about an address – if at all possible choose “accept standardized.”
- You will use the same address with NPPES, WY Medicaid and Magellan.
